Job Title: Inflammatory Bowel Disease Specialized Nurse Job Code: NUR-001 Department: Nursing Services Your Impact - 100% of patients receive an individualized, physician-aligned care plan - 100% of biologic and immunosuppressant therapies monitored per protocol - Seamless coordination across pre- and post-procedure care - Active contribution to program KPI tracking, audits, and quality initiatives - >90% of discharge plans reflect patient and caregiver needs What You Will Do - Patient Assessment: Assess patients, monitor disease activity, and develop individualized care plans. - IBD Management: Administer and monitor biologic therapies, manage flare-ups, and support patients throughout remission. - Patient Education: Educate patients on treatment, self-management, and lifestyle modifications. - Care Coordination: Partner with gastroenterologists and multidisciplinary teams to deliver seamless care. - Documentation & Quality: Maintain accurate documentation while upholding quality, safety, and regulatory standards.
